Eric Bergman, RN BA CCM, is an International Nurse Case Manager for AXAPartners, one of the world’s leading travel assistance companies. At AXA he works in Chicago where he has worked as a chronic disease case manager for US expatriates, a travel assistance case manager overseeing the evaluation and coordination of care all over the world for patients struck by sudden illness while abroad, and as a repatriation escort, using his expertise in both nursing and international travel to safely move ill patients home. After receiving a BA in history from Boston University, Eric worked as a flight attendant and union leader for American Airlines for 27 years. In the early 2000’s, as the airline industry experienced its worst downturn in several decades, Eric made a midlife career change and became an RN. His initial nursing experience was inpatient hospice and oncology where he worked at a community hospital in suburban Chicago for five years. Eric has served on the CMSA Chicago Board of Directors since 2014 and currently serves as Past President of CMSA Chicago, and Presidents’ Counsel Representative for the national organization which includes a seat on the National CMSA Board of directors. Eric also has significant past experience serving on boards of several non-profit organizations, and as a leader of various local and national organizations including as CEO of a not-for-profit educational foundation, treasurer of several service organizations, and manager of local and national political campaigns.

Dr. Colleen Morley DNP RN CMCN ACM is the Director of Case Management for West Suburban  Medical Center in suburban Chicago, IL.   She celebrated her 20th year in Nursing in 2019 and has over 15 years of experience in Case Management in varied roles and settings, including inpatient acute, FQHC Care Coordination and Managed Care. Dr. Morley has been written articles on caseload management and care coordination strategy bets practices.  She has presented projects and educational sessions at local, state and national events, focusing on readmissions reduction and collaborative practice and is active in professional organizations; currently serving as President for CMSA Chicago, Director on the CMSA National Board, and Secretary, Legislative Committee Chair and PAC Trustee on the Board of Directors for ANA-IL.  She launched her own healthcare education consulting firm, Altra Healthcare Consulting, in 2018. Dr. Morley received her Doctorate in Nursing Practice from Chamberlain University, MSN from Norwich University, BSN from Jacksonville University and ADN from South Suburban College.

John Heraty is a Licensed Clinical Social Worker (LCSW), Certified Case Manager (CCM), and is a registered Certified Public Accountant (CPA). He is a telephonic clinical case manager with CVS Health Services/Aetna insurance. He is President Elect-Case Management Society of America (CMSA)- Chicago. He has presented at state and national case management conferences. He is an author. His article contributions can be found in various publications including Washington Post, Wall Street Journal, National Motorist’s Association and two books published by CMSA-Chicago.  He practiced in accounting field for a number of years prior to getting his Master's in Social Work (MSW) from Loyola University of Chicago. His practice field interests are Behavioral Health and crisis counseling, elder & disability rights, patient advocacy and policy issues such as advocating for Nurse Licensure Compact (NLC) and motorists’ rights. John is passionate about advocating for CVS / Aetna’s Medicare members and is big proponent of the power of telehealth.

John is a student of Relational Psychoanalysis and particularly enjoys the work of Stephen Mitchell. He enjoys staying active including long distance running, plyometric exercising, racquetball, political activism and contemplative living. He is an avid writer, blogger & reader and enjoys political discourse & satire.  He is a long time mentor for at-risk youth in Palatine, IL, where he lives with his family.

This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers.  The course is approved for one CE contact hour.
