Learning objectives:

1. Review how changes in healthcare have impacted care needs post discharge.

2. Acquire better understanding of the community-based services available for successful transitioning in the home setting.

3. Understand how risk factors for readmission are identified and how the non-clinical/companion care can make a positive impact.

4. Identify payer options for non-clinical, community-based care and view cost comparisons of different care options.


About the Presenter

Holly Conway-Eaton, Community Outreach Liaison

After a career in leadership roles with healthcare companies (17 years in pharmaceutical sales), Holly decided to turn her passion into her vocation by becoming a caregiver with Senior Home Companions.   In her words, “I wanted to be able to truly impact the lives of the elderly.”   Holly served a number of clients over a four-year period as a caregiver and developed strong relationships with each of them.  Since then, Holly has used her creativity and compassion to help connect SHC with our community. She has been in a marketing position with SHC for over 5 years and has learned that it often does take a village to provide the quality care and resources that enable the elderly to age successfully.  Holly graduated from Ball State University with a degree in Marketing.  She has three children and one granddaughter, who she enjoys spoiling at every opportunity.


This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers.  The course is approved for one CE contact hour.


Community outreach donations for december

CMSA Central Indiana Chapter continues to support the VA Hospital and the Veterans that it serves through our Community Outreach donations program.
For November: we donated 147 pairs of socks, and $175.00 to the programs at VA hospital. This was the wonderful response from our Amazing Members of CMSA Central Indiana and fellow attendees who were present at this year's Fall Conference.

For December: we will be collecting donations of Winter hats, gloves, scarves, and throw blankets to keep the VA patients warm. Please bring your donations to our December Holiday Meeting.

It has been a pleasure to partner with all of the members of our group to give back to those who have given so much.
