A PATHWAY FOR GAINING RECOGNITION OF YOUR KNOWLEDGE AND EXPERTISE IN CASE MANAGEMENT

 

 

Riverside HealthCare Consulting in collaboration with the Central Indiana CMSA Chapter  (3/9) is pleased to offer the following program, A Pathway for Gaining Recognition of Your Knowledge and Expertise in Case Management from 8 AM to 5 PM on November 2, 2019 at St. Vincent Heart Center, Indianapolis, IN.

 

This Review Course is offered as a step in preparation for the CCM examination. It focuses on the five major domains of essential knowledge typically required to provide case management interventions. This base of knowledge includes:

  1. Care delivery and reimbursement methods
  2. Psychosocial concepts and support systems
  3. Quality and outcomes evaluation and measurements
  4. Rehabilitation concepts and strategies
  5. Ethical, legal and practice standards

 

Objectives

Advance the attendee's ability to:

1.Identify core competencies that serve as the foundation of a professional practice of case management.

  1. Discuss key elements necessary for the efficient and effective delivery of and the funding for healthcare services.
  2. Identify the psychosocial concepts that may be incorporated into a professional practice of case management.
  3. Describe quality measures and quality improvement initiatives that are integral to a professional practice of case management.
  4. Apply key ethical, legal and practice standards that guide a professional practice of case management through and across each transition of patient care.
  5. Identify rehabilitation concepts and strategies that impact a professional practice of case management.

 

Target Audience

         Healthcare professionals who are preparing to seek certification in case management from an accepted certification body.

 

Nancy has for the past 25 years served as a national case management educator. In her current role as principal consultant for Riverside HealthCare Consulting, she develops programs that advance excellence in care coordination and other transitional care strategies.

 In 2002, she was named the Case Management Society of America (CMSA) National Case Manager of the Year and in 2008, she received CMSA’s Lifetime Achievement Award. She currently serves as President of the Case Management Society of America and is a Board Member of the Hospital Quality Foundation. Additionally, she assisted in the development of Post –Discharge Standards for Cardiovascular Disease for the Society of Chest Pain Centers.

 Nancy is an Advisory Task Force Member of the National Transition of Care and serves as primary faculty for the University of Southern Indiana Case Management Certificate Course.

 

Number of Continuing Education Hours Offered to Onsite Attendees: 8 (pending approval)

 

Continuing Education Credit is pending through Commonwealth Educational Seminars for the following professions: RNs and SWs

 

As an American Psychological Association (APA) approved provider, CES programs are accepted by the American Nurses Credentialing Center (ANCC). These courses can be utilized by nurses to renew their certification and will be accepted by the ANCC. Every state Board of Nursing accepts ANCC approved programs except California and Iowa, however CES is also an approved Continuing Education provider by the California Board of Registered Nursing (Provider # CEP15567) which is also accepted by the Iowa Board of Nursing.   

 

Commonwealth Educational Seminars (CES) is entitled to award continuing education credit for Social Workers. Please visit CES CE CREDIT to see all states that are covered for Social Workers. CES maintains responsibility for this program and its content.

 

It is the participant's responsibility to check with their individual state boards to verify CE requirements for their state.
