  1. Describe polypharmacy and its potential effects on a patient.
  2. Identify at least three drug classes that could be opportunities for deprescribing.
  3. Assess medications to be deprescribed for patients using online resources that were described.

About the Presenter

Cara Acklin, PharmD., BCACP

Cara is a board-certified ambulatory care pharmacist with experience working in a community pharmacy setting, a hospital onsite outpatient pharmacy, and currently the pharmacist working with the team at the Franciscan Accountable Care Organization. She is passionate about caring for patients ensuring they are taking the most effective medication therapy with the lowest incidence of side effects and a cost they can afford. In her free time, she enjoys spending time with her husband and three kids, camping, attending sporting events - Go Purdue! – and reading.

*This program has been pre-approved by The Commission for Case Manager Certification to provide continuing education credit to CCM® board certified case managers.  The course is approved for one CE contact hour.
