Learning objectives:

  1. Attendee will be able to list and describe tools for managing medications at transitions of care.
  2. Attendee will be able to discuss evidence-based practices for improving medication-related outcomes at care transitions.
  3. Attendee will be able to identify one practice, intervention, or piece of information that can be applied to improve current medication TOC practices at your institution.


About the Presenter

Teresa DeLellis

Teresa is assistant professor of pharmacy practice in transitions of care at Manchester University in Fort Wayne, IN.  As a board-certified geriatric pharmacist, she leads development of geriatric transitions of care programs at Dupont hospital that resulted in accreditation as a Level 1 Geriatric Emergency Department and designation as a Level 3: Senior Friendly NICHE facility.  She is the longest tenured member of the interprofessional leadership team for the region’s Care Transition Coalition, whose successful transitions of care pilot programs have earned national recognition by CMS-contracted Quality Improvement Networks and Organizations. 

Teresa was recently elected board president of her county’s TRIAD organization, also nationally renowned for distinct success in medication take-back programs.  Her program of scholarship intersects across geriatrics, transitions of care, deprescribing, and pharmacogenomics and includes two grant-funded endeavors in their final stages of completion.  Teresa earned her PharmD degree from Wayne State University in Detroit, MI followed by a 2-year Geriatrics residency and Faculty Development Fellowship at UPMC St. Margaret in Pittsburgh, PA.  She now enjoys creating memories and new experiences with her husband, two young boys, and dog Thor in the Summit City of Fort Wayne, donating her time to several community organization boards to cultivate a bright future for her family and community.
